A grounded answer is a reply an AI system produces from a specific, verifiable source rather than from the model's own memory. To "ground" an answer is to tie every claim in it back to supplied material (your documentation, your policies, your product data) so the response reflects what you published, not the model's best guess about it.

Grounding is the direct antidote to hallucination, the tendency of a language model to state false things fluently and confidently. A model answering from general training has no way to know whether its answer is correct for your business; a grounded model answers only from what it was given, and can point to where each part came from. In customer support, where a wrong answer about a refund policy or a medication is a real cost, that difference is the whole game.

Grounded is not the same as retrieved
